If we are not in your area you could easily google “Estate Planning Attorney” near me and I’m sure any of them will be able to help you out with putting this document together. Just know that it’s sometimes offered as part of a total plan however, most firms will break it out and just do a Guardian for Minor Children document for you. It can just get a little confusing if one person quotes an amount a lot higher than another, make sure to ask if it’s for a full plan or one document. :)
